Orbital tumor.

50-year-old male with right orbital tumor, history of dermatofibrosarcoma protuberance.

Procedure: Biopsy.

Frozen section consultation: "Cellular lesion with proliferation of small blue cells, cannot rule out lymphoma" .

Micro: Sections show multiple fragments of soft tissue with conjunctiva epithelial lining and a few submucosal sebaceous glands. There is an extensive infiltration of submucosal neoplastic lymphocytes. The tumor cells are small to medium in size and are characterized by nuclei with highly irregular contours and dense chromatin. Some of the large cells contain nucleoli. These large cells also have moderate amounts of pale stain cytoplasm. H & E stain.

CD 3 Scattered small lymphocytes positive.
CD 5 Lymphoma positive.
CD 10 Negative.
CD 20 Lymphoma positive.
CD 23 Scattered large cells positive.
CD 43 Staining similar to CD 5.
Cyclin D1 Lymphoma positive.
Bcl-2 Lymphoma positive.
Bcl-6 Negative.
Ki-67 The lymphoma has a low to moderate growth fraction (10 to 20% of cells in cycle).
CD 21 Negative.
IgD Lymphoma positive.
IgM Lymphoma positive.
