42 year old female with a left thumb tumor.
Gross: The specimen consists of a pink-tan, lobulated, irregular tissue fragment measuring 2.6 x 2.2 x 1.5 cm. Representative sections are submitted.
Micro: Sections show a well circumscribed and lobulated lesion with partial encapsulation and proliferation of variable portions of mononuclear cells. The tumor exhibits patchy fibrosis and scattered multinucleated giant cells. The mononuclear cells show small, round to spindled nuclei with open chromatin and pale cytoplasm. Occasional nuclear grooves are present. The osteoclast-like giant cells show variable numbers of nuclei ranging from 3 to 50. Occasional mitotic figures are present. There is patchy deposition of siderophages. Areas of stromal hyalinization is also noted. H & E stain.

Pseudoglandular or alveolar like cystic spaces with the giant cells may result in mistakrn diagnosis of alveolar rhabdomyosarcoma .
Giant cell tumour of tendon sheath
Bland round cells, scattered multinucleated giant cells, hyalinized background, cystic spaces, hemosiderin deposition...all of these are features of tenosynovial giant cell tumor, nodular type (aka giant cell tumor of tendon sheath). Interestingly, these are usually at least focally desmin positive, a finding that may be confusing to those who are unaware of this. Great example!
