71 year old female with multiple lung lesions with focal cavitation.
Procedure: CT guided lung lesion needle core biopsy.
Gross: The specimen consists of multiple tan, cylindrical needle core biopsy tissue fragments measuring 0.3 to 1.5 cm. in length, with an average diameter of 0.1 cm. All submitted.
Micro: Sections show cores of tumor tissue with lepidic tumor growth pattern. The tumor cells line up along alveolar septa, project into alveolar spaces with papillary projections, but the underlying lung architecture is preserved. Tumor cells are hyperchromatic with relatively high N/C ratio with pink cytoplasm and prominent nucleoli. Mild nuclear pleomorphism and rare mitotic figures are present. A few tumor cells exhibit intracytoplasmic lumen formation. Apical spouts and hobnail cells are also noted. Focal scar formation is observed in one of the core biopsy tissue fragments. H & E stain
IHC stains:
CK 7 Positive
CK 20 Negative
CDX-2 Negative
TTF-1 Positive
IHC staining pattern is consistent with primary lung cancer.
