40-year-old female with right breast mass.
Procedure: excisional biopsy.
Gross: The specimen consists of an 8 x 5.5 x 2.5 cm. fatty breast parenchyma, weighing 38 gms. There is a firm mass lesion by palpation, and measuring about 3 x 2.5 x 1.5 cm. The specimen is sectioned. The cut surface reveals a tan-white, invasive mass lesion. Representative sections are submitted.
Micro: Sections show invasive cords and nests of tumor cells and prominent lymphoplasmacytic post reactions. The tumor cells are hyperchromatic with high N/C ratio, opened chronic and prominent nucleoli. Frequent mitotic figures and a few bizarre mitotic figures are noted. Tumor cells also show syncytial growth pattern and suggestive of medullary carcinoma. H & E stain

ER Negative (unfavorable)
PR Negative (unfavorable)
Ki-67 Positive (about 90% of tumor, unfavorable)
Her-2/neu < 1+ (not amplified)
