A 22 year-old African male presented with a firm subcutaneous posterior neck mass.

Gross: The specimen is consists of multiple pink-tan, firm, irregular tissue fragments, measuring 0.1 to 0.4 cm. in greatest dimension.

This is a beautiful case with numerous gravid females in cross-section producing microfilariae. The addition of the virtual slide shows the exuberant surrounding inflammatory reaction.

The slides reveal a microfilaria-laden gravid female of Onchocerca volvulus surrounded by fibrous capsule.
It is important to mention that Onchocerca volvulus is the second most common preventable cause of blindness in sub-saharan Africa (called river blindness). This worm can infect eye and skin. In this case it appears in skin. The diagnosis is Onchocercoma.
