70-year-old male with multiple lung masses, liver lesion, and a large kidney mass.
Procedure: CT-guided liver lesion needle core biopsy.
Cytology: Smears of core biopsy with Pap stain.
Gross: The specimen consists of multiple tan, cylindrical, needle core biopsy tissue fragments measuring 0.3 to 1.3 cm. in length with an average diameter of less than 0.1 cm. All submitted.
Micro: Sections show cores of tumor tissue with features of clear cell renal cell carcinoma. The tumor cell exhibits moderate amount of clear cytoplasm and well defined cell membranes. The centrally located tumor cell nuclei are enlarged with hypochromasia, opened chromatin, intranuclear inclusions and prominent nucleoli. Nuclear pleomorphism and irregular nuclear membranes are also noted. The tumor tissue is also rich in vascular networks with dilated capillary sized vessels. Fragments of uninvolved entrapped liver parenchyma are noted with patchy cholestasis, focal mild to moderate portal chronic inflammation, and congested liver sinusoids. H & E Stain.

Comments
The cytoplasmic features are more readily appreciated if DQ staining was performed. You can see the clear cytoplasm in the neoplatic cells better than in pap stain.
