60 -year-old male with left lung mass.
Procedure: CT guided needle biopsy.
Gross: The specimen consists of 2 tan cylindrical needle core biopsy tissue fragments measuring 1.5 and 1.7 cm. in length with average diameter of 0.1 cm. All submitted.
Micro: Sections show cores of lung parenchyma with focal dense fibrosis, chronic inflammation and areas of organizing pneumonia. The adjacent less involved alveolar tissue exhibits interstitial chronic inflammation, reactive type II pneumocytes, and increased intraalveolar macrophages. There is no evidence of malignancy. H & E stain.
Special stains: GMS and AFB stains are negative for microorganisms.
IHC stains:
K-7 Positive, highlighting alveolar lining epithelium.
CD45 (LCA) Positive, highlighting the inflammatory cells.
Ki67 Positive, about 3 to 5%, mainly in inflammatory cells.
