Glioblastoma, WHO grade IV

Most common highly malignant glioma arise most frequently after fifth decade. Secondary GB arises from a grade II or III astrocytoma. Other type is primary or de novo. Occur throughout the brain. Radiology: Contrast enhancing (ring enhancing). Histology: Diverse cytology; monotonus small cell to giant monster cells. Mitotically active. Either of vascular endothelial proliferation or necrosis (w or w/o pseudopalisading) is required for diagnosis. Prognosis: ~1 yr (slightly better in tumor with promotor methylation of MGMT and proneuronal molecular subtype). Rx: Surgery, chemotherapy and XRT
