60-year-old female with recent history of hospitalized because of a mass in her left fifth rib. She underwent wide excision of the rib lesion. She has no fevers or chills. Bone marrow biopsy and aspirate studies were ordered to rule multiple myeloma.
Micro: Bone marrow aspirate smears show active triflange hematopoietic elements with scattered increased plasma cells.
Bone marrow core biopsy shows focal nodular proliferation of plasma cells. The nodular plasma aggregate is about 3 mm. in size. The background bone marrow tissue exhibits trilineage active hematopoietic elements. H & E stain
IHC stains: The nodular plasma aggregate is positive for CD138(Images513 & 515) and Kappa light chain(Image514).
Flow cytometry: Abnormal monoclonal plasma population (1.8%), indicative of plasma cell dyscrasia by flow cytometry immunophenotyping studies.
