Pilonidal cyst
22-year-old male with extensive inflammation, intradermal and subdermal, over the presacral area.
Gross: The specimen consists of a resected, irregular, tan-black skin tissue fragment measuring 11 x 1.5 cm. in size with a depths of 4.5 cm. The skin surface exhibits ulcerative and retracted lesions. The specimen is sectioned. Representative sections are submitted in two cassettes.

Extensive inflammation, intradermal and subdermal, over the presacral area.
Note:
Micro: Sections show dermal cysts with scattered hair shafts and chronic inflammation in the cystic walls. Patchy hemosiderin laden macrophages and a few foreign body giant cells are present. There is no evidence of dysplasia or malignancy. Sections through the old scar reveal abscess formation and extensive granulation tissue proliferation.
