30-year-old female with a polypoid skin lesion in the back.
Procedure: Excisional biopsy.
Gross: The specimen consists of a tan polypoid lesion measuring 1.1 x 1.2 x 0.8 cm. The resection margin is inked blue. The specimen is trisected. All submitted.
Micro: Sections show a polyploid segment of skin with fibrovascular core within which are nests of mature nevus cells. The nevus cells are present along the adnexal structures. Patchy nevus cells are connected and/or converting to spindle shaped cells with wavy nuclei and pink cytoplasm, consistent with neurotization of nevus cells. Sheets of neurotized nevus cells are present in the central portion of the lesion and surrounded by edematous stroma with loose reticulated collagen. The nevus cells are present at the inked resection margin. The epidermis exhibits patchy acantholysis and seborrheic keratosis. H&E stain.
